Elevate Your gives you 500mg per serving.

St John's wort (Hypericum perforatum) is a bright yellow-flowered herb that traditionally blooms around midsummer. When the petals are crushed, they release a red pigment, which is why the plant is also known for its red-tinged oil. It has been used in European herbal medicine for centuries as a nerve tonic and for skin. Today it is taken as tablets, capsules, tea or tincture. It is a potent herb, with many important medicine interactions.

The science behind it

St John's wort contains several active groups. The best known are hyperforin, a prenylated phloroglucinol, and hypericin, a red pigment, along with flavonoids such as quercetin and rutin. Most standardised extracts are labelled for hypericin, hyperforin or both.

Hyperforin appears to inhibit the reuptake of several messengers in the brain, including serotonin, noradrenaline and dopamine, by affecting sodium-dependent transport. It also acts on GABA and glutamate systems. This broad action explains its traditional use for mood and nervous tension, and it is also why it can interact with serotonergic medicines.

Of greater practical importance is its effect on the liver and gut. Hyperforin activates the pregnane X receptor (PXR), which switches on the enzyme CYP3A4, other CYP enzymes and the transporter P-glycoprotein. These systems break down and clear many medicines, so St John's wort can lower the blood levels of many drugs and make them less effective. The effect develops over about two weeks and fades about two weeks after stopping. Low-hyperforin extracts have a lower, but not zero, interaction risk.

A Cochrane review (Linde et al., 2008) examined standardised extracts, and the evidence base is strongest for those. Hypericin can also make skin more sensitive to sunlight. Always tell your pharmacist and doctor if you use this herb.

Common questions

What is St John's wort used for?

It is traditionally used in Western herbal medicine to support mood and emotional wellbeing, ease nervous tension and promote relaxation.

How long does St John's wort take to work?

Studies generally run for four to twelve weeks. Most people do not notice changes in the first week or two, so use it consistently.

When is the best time to take St John's wort?

Take it with food, usually in the morning or split between morning and midday. Some people find evening doses affect their sleep.

Does St John's wort interact with medications?

Yes, it interacts with many common medicines, including the contraceptive pill, blood thinners and many prescription medicines. Always check with your pharmacist or doctor before taking it.

Can I take St John's wort with the contraceptive pill?

It can make the pill less effective and has been linked to breakthrough bleeding and unplanned pregnancy. Speak to your doctor first.

Does St John's wort make you sensitive to the sun?

It can, in some people. Use sun protection and avoid prolonged sun exposure, particularly if you are fair skinned.

Safety

St John's wort has many serious drug interactions: it can reduce the effect of the oral contraceptive pill, warfarin, digoxin, ciclosporin and tacrolimus, HIV medicines, some chemotherapy drugs, statins, theophylline, some seizure medicines, methadone, oxycodone and many others, and it can cause dangerous serotonin syndrome when combined with SSRIs, SNRIs, tramadol, triptans or other serotonergic medicines. Do not take it with any prescription medicine without speaking to your doctor or pharmacist, and do not stop a prescribed medicine to start it. Avoid in pregnancy and breastfeeding, stop at least two weeks before surgery, and use sun protection as it can cause photosensitivity.
