How it works
Saw palmetto (Serenoa repens) is a small palm native to the south-eastern United States. The dark berries were used by Indigenous peoples and later in Western herbal medicine as a tonic, particularly for male urinary and reproductive health.
The active part is a fat-soluble oil extracted from the berries, rich in fatty acids (such as lauric, oleic and myristic acids) and plant sterols such as beta-sitosterol. These compounds are thought to inhibit the enzyme 5-alpha reductase, which converts testosterone to dihydrotestosterone (DHT), a hormone that influences prostate tissue and hair follicles. Extracts are usually standardised to 85 to 95 per cent fatty acids and sterols, and are best absorbed with food. Research results are mixed, and effects build gradually (see the NIH National Center for Complementary and Integrative Health on saw palmetto).

Is saw palmetto only for men?
It is mainly used by men for prostate and urinary support. Some women use it for hair or hormone balance, but evidence is limited. Women who are pregnant or breastfeeding should avoid it.
Safety
Not for use in pregnancy or breastfeeding or by children. Saw palmetto may interact with hormone therapies, contraceptives and blood-thinning medicines. See your doctor about urinary symptoms before self-treating, and tell your doctor you take it before surgery.
