How it works
Garlic (Allium sativum) is a bulb that has been used as both a food and a traditional herbal remedy for thousands of years. Its distinctive smell and many of its effects come from sulfur-containing compounds.
In whole garlic, a stable compound called alliin sits separately from the enzyme alliinase. When a clove is crushed or chopped, they meet and form allicin, which quickly breaks down into other sulfur compounds such as ajoene and diallyl sulfides. These are absorbed and metabolised in the body, and some are excreted through the breath and skin, which explains the lingering odour. Supplements differ: some are standardised for allicin potential, while aged garlic extract contains more stable compounds such as S-allylcysteine. Enteric coating helps protect alliinase from stomach acid (see the NIH National Center for Complementary and Integrative Health on garlic).

Common questions
Does odourless garlic work as well as regular garlic?
Odourless products are often aged garlic extracts or specially processed, and they contain different sulfur compounds from fresh garlic. They are well studied, but are not identical. Choose based on tolerance and the label.
Is it better to take garlic with food?
Most people take it with a meal, which helps avoid heartburn or an upset stomach. Fresh garlic is best crushed and left for a few minutes before eating or cooking, so the active compounds can form.
Safety
Garlic may increase the risk of bleeding, so check with your healthcare professional if you take blood-thinning medicines or are due for surgery. It may also interact with some HIV medicines. Seek advice if you are pregnant or breastfeeding.
